We are seeking a dedicated and proactive pharmacist to join our dynamic team within the inpatient care services. This role presents an excellent opportunity for a motivated individual to contribute to our mission of enhancing patient care through effective medication management.
The successful candidate will play a pivotal role in the delivery of our medication optimization initiatives, collaborating closely with fellow pharmacists and healthcare professionals. The responsibilities will include providing clinical pharmacy services to designated inpatient wards, attending multidisciplinary team meetings, and conducting patient reviews to facilitate informed medication decisions that prioritize personalized care.
Key Responsibilities
The selected candidate will be expected to:
- Collaborate effectively with a range of healthcare professionals to support patient-centered care.
- Engage in the development and enhancement of pharmacy services.
- Maintain up-to-date knowledge of medication optimization practices and contribute to service improvements.
- Provide clinical support to the multidisciplinary team, ensuring high-quality pharmacy services are delivered to inpatients.
As a Clinical Pharmacist, you will be responsible for regular visits to designated wards, working collaboratively with the healthcare team to ensure optimal medication management for patients.
Qualifications and Experience
The ideal candidate will possess:
- A Master's Degree in Pharmacy or equivalent qualifications.
- Registration with the General Pharmaceutical Council (GPhC).
- A Clinical Pharmacy Diploma or equivalent experience.
- Evidence of ongoing professional development.
Experience Requirements
Essential experience includes:
- A solid understanding of various disease states and their treatments.
- A minimum of two years in a clinical pharmacy role.
- Familiarity with medicines legislation and national standards.
- Knowledge of medication management during transitions between care settings.
Additional Information
This position requires travel between various sites, thus a valid driving license and access to a vehicle for business purposes are essential.
