About the Role

We are seeking a compassionate and experienced Clinical Nurse Specialist to join our Community Palliative Care Team at St Christopher's Hospice. As a key member of our interdisciplinary team, you will play a vital role in providing expert care and support to individuals and their families during the last phase of life.

Key Responsibilities
  • Provide symptom control, social, emotional, and spiritual care to patients and their families
  • Work collaboratively with the primary healthcare team and hospital staff to ensure seamless care delivery
  • Develop and implement individualized care plans to meet the unique needs of each patient
  • Conduct telephone and video triage, visiting patients in their homes and providing outpatient services
  • Participate in education and training programs to enhance skills and knowledge in palliative care
Requirements
  • Registered Nurse or Paramedic with experience in end-of-life care
  • Strong communication and interpersonal skills
  • Ability to work efficiently in a multidisciplinary team
  • Resilience and compassion in a challenging and emotionally demanding environment
