Job Summary
The Dorset Clinical Commissioning Group is seeking a skilled and dedicated individual to join our team as a Clinical Assessment and Treatment Service Healthcare Professional. This role will involve working in a community-based setting, providing high-quality care and support to patients.
Main Responsibilities
- Deliver high-quality care and support to patients in a community-based setting
- Work collaboratively with multidisciplinary teams to achieve patient-centered outcomes
- Develop and maintain effective relationships with patients, families, and healthcare professionals
- Contribute to the development and implementation of care plans that meet the needs of patients
- Participate in quality improvement initiatives to enhance patient care and services
Requirements
- Care Certificate or equivalent NVC Level 3 qualification
- Numerate and literate
- Valid car driver's license
- Previous experience working in a community or similar care environment
- Ability to communicate clearly and effectively
- Ability to work as part of a team and manage own time
- Respect for the diversity of patients' values, cultures, and beliefs
Desirable Criteria
- Experience working with adults of all ages
Working Arrangements
This role will involve working in a community-based setting, with regular travel to various locations. The successful candidate will be required to work flexibly to meet the needs of patients and services.
