About the Role
We are seeking a highly skilled and experienced Practice Nurse to join our team at Chiltern House Medical Centre. As a Practice Nurse, you will play a vital role in delivering high-quality patient care and contributing to the success of our multidisciplinary team.
Key Responsibilities
The successful candidate will be responsible for:
- Developing and implementing health promotion and well-being programmes
- Implementing and evaluating individual treatment plans for chronic disease patients
- Identifying, managing, and supporting patients at risk of developing long-term conditions
- Providing routine nursing care to patients as required
- Providing wound care and travel medicine services
- Requesting pathology services and processing results
- Participating in local initiatives to enhance service delivery and patient care
Person Specification
We are looking for a candidate with:
- Registered Nurse qualification
- Experience working in a primary care environment
- Experience in chronic disease management
- Strong clinical knowledge and skills
- Ability to work within own scope of practice and refer to GPs when necessary
