Main Responsibilities- Provide leadership and management to community nursing teams, ensuring the delivery of high-quality patient care and adherence to KPIs, contractual expectations, and CQC requirements.
- Line manage aligned Band 6 and 7 staff, supporting them in their roles and providing guidance and coaching as needed.
- Work closely with other specialist services, GP's, Social services, Hospices, and other MDT partners to provide an integrated approach to patient care.
- Focus on enhancing the quality of patient care, local provision needs, and staff development, while encouraging innovation in care delivery.

Requirements- Registered healthcare professional (NMC registration)
- Health-related degree and evidence of relevant postgraduate development at Level 6

- Strong influencing and negotiating skills, ability to communicate complex information, and highly developed facilitation skills
