This role encompasses conducting comprehensive medication assessments, overseeing chronic disease management, ensuring safe medication practices during transitions of care, and handling prescription renewals and urgent medication requests, all while addressing the health and social care needs of patients.

Key Responsibilities

The responsibilities of this role include, but are not limited to:

  • Conducting thorough medication assessments
  • Reviewing urgent medication requests
  • Managing prescription renewals
  • Processing discharge summaries and clinic correspondence
  • Responding to MHRA safety alerts
  • Addressing inquiries from community pharmacies
  • Enhancing health outcomes for patients with chronic conditions as part of a multidisciplinary healthcare team
  • Performing additional general duties as required
