We are seeking a skilled and dedicated Apprentice District Nurse to join our team at the A31 Alton Primary Care Network. As a key member of our multidisciplinary team, you will play a vital role in delivering high-quality, person-centred care to patients in their own homes.
Main Responsibilities- Develop and implement individualized care plans to meet the unique needs of patients
- Provide expert nursing interventions, including wound management, catheter care, and palliative care
- Work collaboratively with patients, families, and other healthcare professionals to ensure seamless care coordination
- Conduct regular assessments and evaluations to monitor patient progress and adjust care plans as needed
- Registered General Nurse (RGN) qualification
- Current UK/EU driving license and access to a vehicle
- Experience of working in a community healthcare setting
- Ability to work autonomously and as part of a team
- Excellent communication and interpersonal skills
