About the Role:

We are looking for a highly skilled and dedicated Clinical Pharmacist to work in our pharmacy team. The successful candidate will be responsible for:

  • Collaborating with the pharmacy technical team to undertake medicines reconciliation and assessment of patients' own medication.
  • Working within the Medicines Management Policy and procedures to ensure safe and cost-effective medicines use.
  • Discussing prescribed medicines with medical staff to ensure patients receive safe, effective, and appropriate treatment.
  • Monitoring patients' biochemistry and haematology results.
  • Liaising with multi-disciplinary teams to ensure effective continuity of pharmaceutical care.
Requirements:

To be successful in this role, you will need:

  • An undergraduate Master's Degree in Pharmacy.
  • Pre-registration training and successful examination.
  • Pharmacist registration with the General Pharmaceutical Council.
  • 1 or more years of post-registration experience in hospital or community pharmacy.
  • Detailed knowledge and understanding of pharmacy legislation.
