Physiotherapist - Community Care Specialist

We are seeking a highly motivated and experienced Physiotherapist to join our team at Dorset HealthCare University NHS Foundation Trust. As a Band 5 Physiotherapist, you will be responsible for providing high-quality care to patients in the community, working closely with multidisciplinary teams to achieve optimal patient outcomes.
Key Responsibilities- Establish effective communication networks with patients, carers, and multidisciplinary colleagues to contribute to clear decision-making regarding treatment principles and discharge planning.
- Communicate sensitive and unwelcome information regarding patient rehabilitation outcomes and problems surrounding discharge and placement in the community.
- Demonstrate rehabilitation and manual handling techniques to relatives, informal, and formal carers prior to patient discharge, ensuring optimum patient care in the community.
- Liaise and make appropriate referrals to the Dorset Integrated Health and Care System and voluntary partners, to voluntary and statutory services in the community.
- Be responsible for the maintenance of accurate, comprehensive, and contemporaneous patient records detailing all Physiotherapy interventions and significant events, following Trust and departmental policies.

Person Specification- Pre-reg Degree in Physiotherapy (BSc(Hons) or MSc (Hons))
- State Registered (HCPC)
- Member of the Chartered Society of Physiotherapy (desirable)
- Experience working within a care setting (desirable)
- Supervision of assistant staff (desirable)
- UK Licence and use of a car for work purposes (essential)
