About the Role

We are seeking a skilled and compassionate Registered Mental Health Nurse to join our established Community OPMH Team at Havant Health Centre. As a Community Mental Health Nurse, you will play a vital role in providing high-quality care to service users with severe and enduring mental illness.

Main Responsibilities

  • Act as a care coordinator to an agreed caseload of service users, providing ongoing assessment, treatment, and evaluation.
  • Undertake initial and follow-up assessments, and develop and implement individualized care plans.
  • Supervise and mentor junior staff and students, promoting a culture of excellence and continuous learning.
  • Work collaboratively with other healthcare professionals to ensure seamless care coordination and effective service delivery.
