Position Overview:
As a Senior Care Coordination Manager, you will play a pivotal role in overseeing tailored support packages for individuals with diverse needs. This position is primarily field-based, requiring travel across the designated region while also allowing for remote work.

Key Responsibilities:
1. Assessment and Care Planning:
- Conduct comprehensive evaluations to identify the unique needs of each client.
- Create and implement personalized care strategies that encompass medical, emotional, and social aspects.
- Regularly review and modify care plans to ensure they are effective and relevant.

2. Support Worker Induction:
- Develop and facilitate thorough onboarding programs for new support staff, ensuring they are well-versed in company protocols and the specific needs of clients.
- Provide initial training on specialized care techniques and equipment.

3. Team Supervision:
- Offer continuous mentorship and support to care staff, fostering a collaborative and productive environment.
- Conduct performance evaluations and provide constructive feedback to enhance team development.

4. Client and Stakeholder Engagement:
- Organize and participate in regular meetings with clients, families, and integrated care boards to ensure effective communication and care coordination.
- Address feedback and concerns to improve service quality continuously.

5. Quality Assurance:
- Perform unannounced checks to ensure compliance with care plans and company standards.
- Provide immediate support and guidance during spot checks to uphold care quality.

6. Incident Management:
- Respond promptly to incidents, ensuring the safety and well-being of all parties involved.
- Conduct thorough investigations to identify causes and prevent future occurrences.

7. Reporting and Documentation:
- Prepare detailed reports on client progress and care status, ensuring timely communication with stakeholders.
- Maintain accurate records in compliance with regulatory standards.

Qualifications:
- Exceptional communication skills, both verbal and written, with a focus on clarity and empathy.
- Proven experience in managing care for clients in community settings.
- A genuine passion for delivering high-quality, person-centered care.
- Strong organizational skills with the ability to prioritize effectively.
- Proficiency in Microsoft Office and adaptability to new software systems.
- A valid UK driving license and access to a vehicle for travel.
- Ideally, hold an NVQ Level 3 in Health & Social Care or a willingness to pursue further qualifications.
