Position Overview
The role involves delivering a proactive and responsive case management function within the Integrated Discharge service. This position serves as a pivotal point for managing intricate cases at the ward level, contributing to the evaluation of needs, discharge strategies, and the coordination and assessment of services necessary for the safe transition of patients from hospital settings.
As a member of the Integrated Discharge service Team, the individual will play a crucial role in facilitating efficient and effective discharge processes for adult patients across various hospital wards. This will be conducted under the supervision of Complex Case Managers, with the aim of minimizing patient length of stay and ensuring that discharge plans are dynamic and effectively progressing from the point of admission.
Key Responsibilities
- Provide proactive and responsive assistance to team members within the integrated discharge framework.
- Foster and sustain positive collaborative relationships with system partners and ward staff.
- Primarily based within the Integrated Discharge service Team, with responsibilities extending across multiple hospital sites to fulfill service requirements.
- Operate independently, utilizing initiative without direct oversight when off-site, while keeping Complex Case Managers informed of progress and any arising issues.
- Ensure that patient case documentation and referral information are managed in accordance with organizational policies and accurately recorded.
- Maintain a confidential and professional demeanor at all times, engaging with patients, families, colleagues, and health professionals through various communication methods.
- Participate in gathering and analyzing information related to delayed discharges, collaborating with ward coordinators to initiate alerts for qualified team members to address these delays.
- Provide guidance to ward personnel regarding discharge processes as questions arise within the scope of knowledge and responsibilities.
- Escalate unresolved issues and delays in care to senior management to facilitate timely resolutions.
- Prioritize and manage a personal caseload, conducting daily ward visits and ensuring high standards of service delivery.
- Support discharge processes that align with established operational policies.
- Direct any complaints or unresolved matters to the appropriate line manager or designated individual.
- Respond promptly and professionally to communications and alerts.
- Screen and input data on referrals and assessment documents.
- Identify risk factors within referral documents and take appropriate action.
- Work within the limits of personal competence, referring complex issues to qualified team members.
- Utilize various electronic systems for tracking referrals and patient data.
- Provide additional support to the Integrated Discharge service as needed, including training and coverage for absences.
- Recognize personal limitations and areas of responsibility.
- Document accurate records of activities undertaken, highlighting any deviations from expected outcomes.
- Compile and maintain monthly performance statistics as required.
Essential Qualifications
- A solid educational background, including English and Mathematics at a minimum of GCSE level 4-9.
- NVQ or equivalent training/experience in an acute healthcare environment.
- Experience in ward-based discharge planning.
- Proven experience in working collaboratively within a multidisciplinary team.
- Background in healthcare services.
- Ability to manage a diverse workload with competing priorities while meeting tight deadlines.
