We are seeking a highly motivated and compassionate Community Mental Health Nurse to join our esteemed Older Adult Team.

In this role, you will play a vital part in providing exceptional care to individuals aged 65 and above experiencing mental health challenges. Your responsibilities will encompass:

  • Clinical Expertise: Collaborate with Senior Practitioners (CPN) to deliver comprehensive patient care, encompassing assessment, care planning, implementation, and evaluation.
  • Patient-Centered Approach: Work in partnership with patients, families, and caregivers to develop personalized care plans that address their unique needs and goals.
  • Medication Management: Monitor medication regimens and liaise effectively with psychiatrists to ensure optimal patient outcomes.
  • Interdisciplinary Collaboration: Actively participate in a multidisciplinary team (MDT) environment, fostering strong relationships with primary care providers, social services, and other relevant stakeholders.
  • Mentorship and Supervision: Provide guidance and support to nursing students and Support Workers as assigned.

Essential Requirements:

  • Registration with the NMC and adherence to the Code of Conduct.
  • Valid driving license; access to a vehicle for work purposes is essential.
