Reablement Officer

Job Summary: We are seeking a skilled Reablement Officer to join our team at Service Care Solutions. The successful candidate will work closely with our clients to enable them to achieve maximum skills for independent living in the community.

Key Responsibilities:

  1. Support the team leader in ensuring that individuals receive person-centered care and support to regain their independence.
  2. Provide direct management support to support workers, promoting a restorative approach to maximizing an individual's independence.
  3. Develop and implement goal plans and intervention plans that focus on outcomes, ensuring a seamless service delivery.
  4. Collaborate with healthcare professionals to provide specialist reablement assessments and develop tailored support plans.

Requirements:

  1. Good understanding of current health and social care policies, with the ability to apply this knowledge in a practical setting.
  2. Ability to develop and implement effective support plans, promoting independence and empowerment.
  3. Level 2 or 3 NVQ or other form of comparable training, demonstrating a strong foundation in care and support principles.

Working Arrangements: This role will work 2 shifts on a rolling rota, including weekend and bank holidays. The successful candidate will be required to work flexibly to meet the needs of our clients.
