Job Summary

We are seeking a highly skilled Clinical Pharmacist to join our team, working across our Earby and Colne GP surgeries. As a Clinical Pharmacist, you will be responsible for providing medical services to our patient population, managing a caseload, and dealing with a wide range of health needs.

Main Responsibilities

  • Signing of EPS repeat prescriptions, ensuring all monitoring and reviews are up to date
  • Review and issue prescription requests
  • Assist with medication requests and queries
  • Assist with acute medications, reviewing length of time taken and suitability for repeat prescribing
  • Signing of weekly and fortnightly prescriptions, ensuring appropriate issue dates for collection from pharmacy
  • Requests for alternative medications due to manufacturing unavailability
  • Supporting prescription query team and administration team with prescription queries
  • Medication Reconciliation - reconciling medications in line with hospital discharges and clinic letters
  • Care Homes - assisting with and supporting medication queries and liaising with existing ELHT care home SMOCH team and PCN team
  • Safety in Prescribing - assisting with and supporting safe prescribing for high-risk drugs and implementing drug safety alerts
  • Prescribing Initiative Scheme - assisting with and supporting implementation and actions to achieve targets for the scheme
  • Training - assisting with and supporting training for new staff, clinical and non-clinical
  • Medication Reviews - assisting with routine medication reviews and structured medication reviews for identified patients

Expanded Responsibilities

As a Clinical Pharmacist, you will work closely with the Lead Pharmacist to expand your responsibilities and contribute to the development of our medication management services.

Person Specification

  • Mandatory Registration with the General Pharmaceutical Council
  • Degree in Pharmacy
  • MPharm or equivalent
  • About to complete Independent Prescriber status
  • Experience of working in a healthcare organization
  • Experience of working in Primary Care or GP Practice
  • Independent Prescriber or about to complete
