Job Title: Community / District Nurse

We are seeking a skilled and compassionate Community / District Nurse to join our team at Care By Us Ltd. As a key member of our healthcare team, you will play a vital role in delivering high-quality care to adults, children, and young people with varying conditions.

Job Summary

This is a field-based role, where you will manage your own diary and maintain a work-life balance while giving your clients the opportunity to enjoy a quality life in their own home. You will be assessing and reviewing individual care packages, providing practical training to care workers, clinical supervisions, and instructions using your clinical expertise and guidance.

Key Responsibilities
  • Assess and review individual care packages to ensure the best possible outcomes for clients
  • Provide practical training to care workers to enhance their skills and knowledge
  • Conduct clinical supervisions and provide instructions to care workers using your clinical expertise and guidance
  • Work with healthcare workers at the point of care, including home, school, community, and day centers
  • Interact with ICBs, Case Management, Social Services, private and NHS hospitals, nursing homes, and learning disability establishments
  • Participate in our paid phone support On Call Rota on a rotational basis
