We are seeking a highly skilled and compassionate Resilience Specialist to join our team at Change Grow Live Nottinghamshire. As a key member of our Multi-Disciplinary Team, you will play a vital role in supporting young adults and young people in supported accommodation who are struggling with substance use issues.
Key Responsibilities- Engage and Support Young Adults and Young People: Provide them with relevant information, advice, and interventions to support their health and wellbeing needs, including facilitating access to clinical interventions.
- Complete Comprehensive Assessments: Develop individual care plans that incorporate integrated care and transition support to address the person's substance use and related outcomes.
- Safeguarding and Advocacy: Demonstrate commitment to information governance processes and safeguarding principles, advocating and empowering young adults and young people to access partnership and community services.
- Wellbeing and Resilience: Enable them to lead safe, healthy, and purposeful lives, reducing risk and increasing resilience.
- Experience Working with Young People: Excellent knowledge of the issues they face, with a strong understanding of the challenges and opportunities in this field.
- Safeguarding and Substance Misuse: Knowledge of safeguarding concerns related to children and young people, as well as the Fraser Competence framework, and a deep understanding of drug and alcohol issues affecting young people and young adults.
- Teamwork and Interpersonal Skills: Excellent team working and interpersonal skills, with the ability to maintain a highly cooperative approach to supporting colleagues in delivering service objectives.
