Job Summary

We are seeking a highly skilled and compassionate Primary Care Nurse to join our team at South West Yorkshire Partnership NHS Trust. As a Primary Care Nurse, you will be responsible for delivering high-quality nursing care to patients in home and community-based settings.

Key Responsibilities
  1. Delivering Skilled Nursing Care: Provide skilled nursing care to patients in the community setting, including recording and monitoring clinical observations, wound care management, and medicines management.
  2. Planning and Delivery of Care Packages: Participate in the planning and delivery of packages of care, including liaison with other health professionals and statutory and voluntary agencies.
  3. Liaison and Communication: Liaise between patients/carers and other agencies, including arranging and coordinating multi-disciplinary team meetings to evaluate patient care.
  4. Clinical Skills and Autonomy: Demonstrate a range of clinical skills that support high-quality person-centered care for the caseload population, working autonomously in a variety of community settings.
  5. Record Keeping and Data Entry: Record all patient contacts and activities accurately and contemporaneously, maintaining accurate records and statistical returns as required by the Trust.
  6. Telephone Enquiries and Advice: Respond to telephone enquiries from the general public and residential and nursing home staff, providing advice or attending the patient to carry out an assessment and/or treatment.
  7. Communication and Partnership Working: Establish and maintain excellent relationships with Health and Social Care professionals, initiating referrals where appropriate to ensure adequate support for patients and carers.
  8. Culture of Openness and Recognition: Lead and foster a culture of openness and recognition of duty of candour, inspiring a shared purpose to support the delivery of high-quality effective care.
  9. Service Improvement and Development: Contribute to the development, collation, monitoring, and evaluation of data relating to service improvement and development, quality assurance, quality improvement, and governance.
  10. Clinical Governance and Health and Safety: Work within the clinical governance framework to maintain consistent high standards of care, working from an evidence base and current best practice, and implement health and safety policies.
  11. Mentorship and Training: Act as a mentor for students and participate in all aspects of and levels of training to consolidate theoretical and practical teaching of staff and learners.
  12. Clinical Supervision: Participate in Clinical Supervision within the team on a regular basis.
