Primary Care Nurse for Care Homes

We are seeking a highly skilled and dedicated Primary Care Nurse to join our team in the Neighbourhood Rehabilitation Service. As a key member of our team, you will be responsible for delivering high-quality nursing care to patients in their own homes and community-based settings.
Key Responsibilities- Assess, plan, implement, and evaluate care for patients in the community.
- Work in partnership with other health professionals to provide holistic patient care.
- Develop and maintain effective relationships with patients, carers, and other healthcare professionals.
- Contribute to the development of service improvement and quality assurance initiatives.
- Participate in training and professional development to maintain and enhance clinical skills.

Requirements- Current registration with the Nursing and Midwifery Council (NMC).
- Experience in community nursing, preferably in intermediate care.
- Excellent communication and interpersonal skills.
- Ability to work flexibly and autonomously in a community setting.
- Valid driving license and access to a car during working hours.
