We are seeking a highly motivated and experienced Band 6 nurse to join our Chronic Kidney Disease multidisciplinary team on a 12-month secondment to cover maternity leave.
Main Responsibilities:- Act as the key worker and liaise with other healthcare professionals to provide specialist care for patients.
- Provide high-quality nursing care to patients with chronic kidney disease.
- Develop and implement individualized care plans to meet the needs of patients.
- Collaborate with the multidisciplinary team to ensure seamless care delivery.
- Current NMC registration.
- Minimum of 2 years post-registration experience in Renal nursing.
- Formal leadership and ENB 998 or equivalent.
- Current driving license.
