About the Role
We are seeking an experienced Community Caseload Manager to join our multidisciplinary team at East Cheshire NHS Trust. As a key member of our team, you will be responsible for providing holistic, high-quality, safe, and skilled health and social care/support to patients, carers, and their families.
Main Responsibilities
- Work closely with members of the Integrated Care Team to provide rapid and planned expert professional assessment and treatment interventions for patients in their own homes or other community settings.
- Manage individual and team caseloads, carrying continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care programs.
- Provide leadership and support across the care community team, demonstrating emotional intelligence to recognize pressures of self and team.

Requirements
