Care Team Manager Role at Care UK

We are seeking a compassionate and dedicated individual to lead our care team as a Care Team Manager. As a key member of our team, you will be responsible for supervising and leading your shift, managing residents' cases, and collaborating with families, healthcare professionals, and other stakeholders to ensure optimal care plans are developed and implemented.

  • Supervise and lead your shift to ensure seamless care delivery
  • Manage residents' cases and develop personalized care plans
  • Collaborate with families, healthcare professionals, and other stakeholders to ensure effective care coordination
  • Administer and document medication as required
  • Conduct risk assessments to identify potential care needs
  • Assist care assistants in supporting residents with daily requirements, promoting their independence
  • Collaborate with the leadership team to enhance care services and improve resident outcomes

About You

We are looking for an individual with a strong background in healthcare and social care, preferably with a SVQ Level 3 in health and social care. You should be organized, IT savvy, and adept at assigning and delegating tasks efficiently. Demonstrated leadership and supervision skills, as well as experience in care planning processes and medication administration, are essential for this role.
