Job Title: Autonomous Practitioner - District Nurse
Job Summary:
We are seeking an experienced and skilled Autonomous Practitioner to join our Community Nursing Team at Your Healthcare CIC. As a key member of the team, you will be responsible for providing high-quality patient care and leading the development of our district nursing service.
Key Responsibilities:
- Assess the health needs of clients and develop comprehensive care plans to promote choice and empowerment.
- Monitor and maintain a high standard of clinical practice, both for self and other nursing staff within the team.
- Develop and maintain links with the district nursing service and promote integrated working with other community agencies.
- Provide care for patients requiring intravenous therapy, subcutaneous therapy, peg feeds, or other appropriate treatments.
- Plan and implement care that aims to prevent avoidable hospital admission.
- Optimize opportunities to provide care in patients' preferred place, including terminal care.
- Maintain professional competence and personal development through participation in mandatory and appropriate training.
- Responsible for the maintenance of timely and accurate written and electronic patient records.
- Demonstrate understanding of clinical governance and risk management and integrate this into practice.
- Participate in a supervision and appraisal process and ensure the development and maintenance of a personal development plan.
- Facilitate a learning environment for pre and post-registration nursing students and students from other health and social care settings.
- Participate in and ensure team compliance in the collection of statistical information which informs performance data for the team.
- Promote integrated team working and adherence to YHC policies.
- Effectively communicate relevant local and national initiatives to the team.
- Practice at all times within a clinical governance framework that promotes quality.
- Provide line management for the team of qualified and unqualified nursing staff.
- Undertake supervision and appraisals of team members to develop personal development plans and contribute to team and personal objectives.
- Participate in the development and integration of care pathways, policies, and procedures.
- Support an environment that is patient-focused and enables the nursing team to challenge all aspects of patient care.
- Be a role model through the exercise of good personal time management, punctuality, consistent reliable attendance, and high-quality care provision.
- Establish robust and effective communication networks and working relationships with the wider district nursing service.
- Develop and maintain communication links with ward staff, promoting the importance of community team involvement in discharge planning.
Requirements:
- Registered Nurse with a valid NMC registration.
- Experience in district nursing or a related field.
- Strong leadership and management skills.
- Excellent communication and interpersonal skills.
- Ability to work autonomously and as part of a team.
- Knowledge of clinical governance and risk management principles.
- Ability to maintain accurate and timely patient records.
- Willingness to participate in ongoing professional development.
