Position Overview

We are seeking a dedicated and qualified professional to become a vital member of our supportive Intermediate Care Team.

This is a full-time role (hours) scheduled from Monday to Friday, with the potential for future adjustments to a 7-day operational model.

The successful candidate will provide assistance to qualified therapists in both inpatient settings and Intermediate Care throughout the Wigan borough, and may also be involved in supporting the New Intermediate Care at Home service within community environments.

It is crucial for this position that you possess the capability to travel independently across the borough to evaluate, develop therapy plans, and implement therapeutic interventions for patients in their residences.

Key Responsibilities

The primary objective of the inpatient areas is to avert hospital admissions or facilitate earlier discharges, enabling patients to reach their maximum functional potential for a safe return home or to a preferred location.

The ideal candidate will have experience working with patients facing a range of medical, functional, and social challenges, along with strong communication and teamwork abilities.

Job Duties

Planning and Organizational Responsibilities

  • Assist the therapy service in evaluating and treating patients with a diverse array of complex clinical conditions.
  • Utilize judgment skills to assess and evaluate patient conditions, making necessary adjustments to existing treatments or advancing a patient's rehabilitation program within established clinical protocols.
  • Conduct initial assessments to gather information for therapists.
  • Support the Community REACT Team in delivering an effective service to patients in alignment with overall service policies.
  • Administer exercise programs to patients, either individually or in group settings, with or without direct supervision from therapists, including the development and modification of treatment plans as required.
  • Manage a designated patient caseload, including basic evaluation and progression of treatments, and report changes and responses to treatment to qualified therapists.
  • Maintain close communication with supervising therapists, updating them on significant changes in patient conditions to facilitate treatment modifications.
  • Promote safe moving and handling practices tailored to patient needs.
  • Ensure all patients are treated with dignity, focusing on their comfort, wellbeing, and safety, and handle sensitive situations with care.
  • Assist patients as needed before, during, and after treatment.
  • Maintain patient confidentiality in accordance with local and national guidelines.
  • Work independently in various settings, with telephone support available at all times.
  • Plan and prioritize workload in accordance with service agreements.
  • Adhere to organizational and departmental standards, policies, and procedures.
  • Engage in continuous personal development and maintain competency to practice at the Rehabilitation Practitioner level.
  • Communicate the role and purpose of the Rehabilitation Practitioner to patients, caregivers, and others.
  • Promote the services provided to users and colleagues.
  • Participate in the development and updating of departmental resources.
  • Contribute to clinical governance activities within the department.
  • Work flexibly as directed by service needs.
  • Attend all mandatory training sessions.
  • Ensure all physiotherapy equipment is maintained in a clean and safe condition.
  • Report any faulty equipment to the line manager immediately.
  • Support the work of Physiotherapists and Occupational Therapists in managing patients requiring rehabilitation.

Communication and Key Relationships

  • Establish rapport with clients before applying technical skills.
  • Communicate information clearly to patients, caregivers, and others.
  • Understand communication barriers for patients with cognitive impairments and use effective communication to encourage patient engagement in rehabilitation.
  • Collaborate effectively with other members of the multidisciplinary team.
  • Act as a liaison between therapists, patients, families, and caregivers.
  • Maintain accurate records and documentation.
  • Attend relevant care planning meetings and multidisciplinary team meetings as required.

Health & Safety Responsibilities

In compliance with the Health & Safety at Work Act 1974, the post holder is expected to proactively manage risks in all actions, including conducting risk assessments and reporting incidents or hazards.

Teaching Responsibilities

  • Maintain competency through continuous professional development and reflect on previous experiences.
  • Participate in sharing information and skills with students and staff, including supervising and training new assistants.

Qualifications and Experience

Essential

  • Foundation Degree/Level 5 Assistant Practitioner Qualification or equivalent Band 4 experience with a CPD portfolio.
  • Experience working with clients in community or intermediate care settings.
  • Experience conducting independent assessments and implementing interventions.
  • Experience in falls prevention, mobility, and balance issues.
  • NHS experience.
