Senior Health Care Support Worker

We are seeking a highly motivated and experienced Senior Health Care Support Worker to join our Community Learning Disability Health Team in Barnsley. This is a full-time Band 3 permanent post.
Main DutiesThe post holder will work under the supervision of registered health care professionals to deliver planned programmes of care and treatment to designated service users. Key responsibilities include:
- Providing support to service users to understand their health needs and promoting annual health checks
- Completing hospital passports and providing health action plans
- Supporting service users to health appointments and providing health education using easy read resources
- Working closely with carers and families in various care settings
- Managing a caseload under the supervision of a named nurse
The post holder will be responsible for:
- Delivering excellent levels of customer service to people
- Assessing, problem-solving, or reporting when customer service standards have not been met
- Working with a degree of independence within defined parameters
- Contributing to the initial assessment, admission, or discharge process and formulation of individualized packages of care
- Acting as a link or associate link for a specific area of practice
- Clearly, accurately, and contemporaneously contributing to care documents, reporting observations, and other service user information
The ideal candidate will possess:
- GCSE A-D or grade 3-9 in Maths and English or equivalent
- Apprenticeship level 3 or equivalent, such as RQF NVQ level 3 in Health & Social Care
- Or, level 2 qualification with the ability to demonstrate competencies at level 3
- Care Certificate or required to achieve within 12 weeks of commencement
The post holder will be required to undertake all appropriate Trust mandatory training requirements and subsequent refresher training relevant to the area.
