About the Role

We are seeking a highly skilled and motivated Rotational Children's Community Staff Nurse to join our team at Solent NHS Trust. As a key member of our Community Children's Nursing Service, you will play a vital role in providing high-quality nursing care to children and young people with short-term acute health care needs.

Key Responsibilities
  • Support the Team Leader in carrying out specific health care assessments and/or interventions.
  • Carry out initial assessments and triage of patients with short-term health care needs by telephone, at clinical settings, or in patients' homes under the supervision of a Team Leader.
  • Provide continuity of care for children on caseload and maintain contemporaneous records accordingly.
  • Discharge patients according to pathways of care, under the supervision of a Team Leader.
  • Support the provision of Community Children's Nursing service within special schools as service needs require, under the supervision of a Special School Children's Nurse.
  • May carry out initial home assessments with guidance from the Team Leader, to plan, implement, and evaluate programmes of care.
  • Work in partnership with families and the multi-disciplinary team to provide care in appropriate settings to maximise life opportunities for the child or young person.
  • Report any changes in the child's condition to the Team Leader and medical colleagues and ensure all records are maintained in accordance with Trust policies.
  • Participate in the provision, storage, and administration of medicines, including intravenous therapy, in accordance with Service/Trust guidelines and policies, and in partnership with pharmacy colleagues.
  • Participate in MDT meetings to support children with complex health care needs.

Person Specification
  • RN Child qualification.
  • Evidence of ongoing development.
