Palliative Care Clinical Nurse Specialist - Community Team

We are seeking a highly skilled and compassionate Palliative Care Clinical Nurse Specialist to join our Community Team at Woking & Sam Beare Hospices. As a key member of our team, you will provide specialist care and support to patients with life-limiting illnesses and their families in the community.

Key Responsibilities:
  • Provide specialist knowledge and expertise in caring for patients with life-limiting illnesses and disseminate such knowledge to the healthcare team.
  • Independently manage own caseload of patients and associated responsibilities.
  • Assess, plan, implement, and evaluate all aspects of patient care.
  • Provide symptom control, social, emotional, and spiritual care for patients, families, and carers.
  • Monitor patients on an ongoing basis, evaluate outcomes, and adjust care plans to ensure optimum coordination and delivery of care.
  • Deliver high standards of care directly and assist in the management of junior staff.
  • Develop and maintain effective relationships with all members of the multi-disciplinary team to ensure collaborative working for the benefit of patient care.
Requirements:
  • Registered Nurse
  • Relevant post-registration courses
  • Palliative care qualification
  • 1st level degree or masters course
  • Independent Nurse Prescriber
  • Management/leadership course
  • Mentorship/teaching qualification
  • UK Registration
  • Disclosure and Barring Service Check
