About the Role

We are seeking a highly skilled Clinical Nurse Specialist to join our Heart Failure Team at University Hospitals of Morecambe Bay NHS Trust. The successful candidate will work closely with our multidisciplinary cardiology team to ensure seamless transfers of care for patients with heart failure.

Main Responsibilities
  • Assess, plan, and implement care plans for community patients with heart failure.
  • Work collaboratively with the acute heart failure team to ensure effective transitions of care.
  • Develop and maintain strong relationships with patients, families, and healthcare professionals.
  • Contribute to the development and implementation of heart failure services and policies.
Requirements
  • 1st Level Registered Nurse qualification.
  • V300 Prescriber or willingness to work towards.
  • Clinical Examination skills or willingness to work towards.
  • Heart Failure or Long-Term Condition study or willingness to work towards.
