Nurse Associate

We are seeking a skilled and compassionate Nurse Associate to join our Primary Care Network team. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality patient care and contributing to the delivery of integrated healthcare services.
Main Responsibilities- Work collaboratively with our team to provide and monitor patient care, under direct or indirect supervision.
- Contribute to the improvement of patient safety and quality of care at every opportunity.
- Support registered Practice Nurses in delivering patient care.
- Develop relationships across the multidisciplinary team to support the integration of healthcare services.
- Perform and record clinical observations, such as blood pressure, temperature, respirations, and pulse.
- Provide flu vaccinations, ECGs, and venepuncture, and other relevant clinical tasks as required by the Primary Care Network, after undertaking additional training.
- Promote health and well-being to all patients, including undertaking the NHS health check.
- Care for individuals with complex conditions, such as dementia, mental health conditions, and learning disabilities.
- Advise patients on general healthcare and promote self-management, including signposting patients to personalized care colleagues and local community and voluntary sector services.
- Maintain accurate and contemporaneous patient health records.
- Enhance own performance through continuous professional development, imparting own knowledge and behaviors to meet the needs of the service.
Experience
Essential
- Experience of working within a nursing team.
- Experience of working in Primary Care.
Desirable
- Experience working in a GP Practice.
- Clinical system experience - System 1.
Qualifications
Essential
- Registered Nursing Associate on the NMC register.
- Nursing Associate foundation degree.
- GCSE Grade A-C in Maths & English.
