About the Role

We are seeking a skilled and compassionate Nursing Associate to join our Primary Care Network healthcare team at Wentworth 1 PCN. As a key member of our multidisciplinary team, you will play a vital role in providing high-quality patient care and contributing to the delivery of integrated healthcare services.

Key Responsibilities

  • Work collaboratively with our team to provide and monitor patient care, under direct or indirect supervision.
  • Improve safety and quality of care through proactive measures and continuous learning.
  • Contribute to the development of integrated care plans and support registered Practice Nurses in their roles.
  • Develop relationships across the multidisciplinary team to enhance integration of care across health and social care settings.
  • Perform and record clinical observations, such as blood pressure, temperature, respirations, and pulse.
  • After additional training, provide flu vaccinations, ECGs, and venepuncture, and other relevant clinical tasks as required by the PCN.
  • Promote health and well-being to all patients, including undertaking NHS health checks.
  • Care for individuals with complex conditions, such as dementia, mental health conditions, and learning disabilities.
  • Advise patients on general healthcare and promote self-management, including signposting to personalized care colleagues and local community services.
  • Maintain accurate and contemporaneous patient health records.
  • Enhance own performance through continuous professional development and share knowledge and best practices with the team.

Person Specification

Essential

  • Experience of working within a nursing team.
  • Experience of working in Primary Care.

Desirable

  • Experience working in a GP Practice.
  • Clinical system experience, including System 1.

Qualifications

  • Registered Nursing Associate on the NMC register.
  • Nursing Associate foundation degree.
  • GCSE Grade A-C in Maths & English.
