About the Role

We are seeking a highly skilled and dedicated Social Worker to join our Resilience Services team at South Gloucestershire Council. As a key member of our team, you will play a vital role in supporting vulnerable adults and ensuring they receive the best possible care.

Key Responsibilities
  • Conduct thorough assessments and develop personalized care plans to meet the needs of service users.
  • Work closely with our Home to Decide Care Responders to provide reablement support and motivate service users to regain confidence and independence.
  • Review and update care plans, providing guidance and feedback as necessary.
  • Collaborate with multi-disciplinary teams to ensure effective case management and safeguarding.
  • Ensure accurate and timely record-keeping, adhering to professional standards.
  • Undertake assessments of capacity and best interest decisions, as required, under the Mental Capacity Act.
Requirements
  • Relevant professional qualification, such as a Social Work Diploma or equivalent, and registration with Social Work England.
  • Relevant post-qualifying experience in social care or a related field, or transferable skills and knowledge.
  • Ability to provide evidence of ongoing continuous professional development, knowledge of current legislation and practice, and risk assessment and management skills.
  • Excellent prioritization, time management, and communication skills.
Advanced Social Worker Requirements
  • Minimum of 2 years post-qualification experience.
  • Evidence of meeting the essential and desirable criteria for the role of Advanced Social Worker or in South Gloucestershire Council.
  • Clear commitment to Continuous Professional Development (CPD).
