The Medical People Recruitment Ltd is seeking a highly skilled and experienced Clinical Lead Dietician to join our team in a hospital in Wales. As a key member of our community integrated care team, you will be responsible for leading the development and implementation of clinical dietetic services.
Key Responsibilities:
- Provide specialized clinical dietetic services to the acute and urgent response team and surrounding integrated community teams.
- Lead on training, education, and development of support workers and the wider community teams.
- Embed current policy and best practice to take early action on malnutrition by reducing hospital admissions and improving patient outcomes.
- Collaborate with multidisciplinary teams to develop and implement effective care plans.
- Conduct needs assessments and develop personalized care plans for patients.
- Monitor and evaluate the effectiveness of clinical dietetic services and make recommendations for improvement.
Requirements:
- Registered Dietitian with a strong background in community integrated care.
- Proven experience in leading clinical dietetic services and developing training programs.
- Excellent communication and interpersonal skills.
