Practice Nurse

We are seeking a highly skilled and experienced Practice Nurse to join our team at Chiltern House Medical Centre. As a Practice Nurse, you will play a vital role in delivering high-quality patient care and contributing to the success of our primary care services.
Main ResponsibilitiesThe successful candidate will be responsible for:
- Developing and implementing health promotion and well-being programs
- Implementing and evaluating individual treatment plans for chronic disease patients
- Identifying, managing, and supporting patients at risk of developing long-term conditions
- Providing routine nursing care to patients, including wound care and travel medicine services
- Requesting pathology services and processing results
- Providing chronic disease clinics and referring patients to secondary/specialist care as needed
- Maintaining accurate clinical records and ensuring read codes are used effectively
- Developing and implementing well woman and well man clinics
- Chaperoning patients and assisting GPs with minor surgery
- Prioritizing health issues and intervening appropriately
- Supporting the team in dealing with clinical emergencies and recognizing, assessing, and referring patients with mental health needs
- Implementing vaccination programs and supporting patients with prescribed medicines
- Liaising with external services and agencies to ensure patient support
- Delegating clinical responsibilities and supporting the clinical team with safeguarding matters
In addition to the primary responsibilities, the Practice Nurse may be requested to:
- Act as the audit lead and effectively utilize the audit cycle
- Monitor and ensure the safe storage, rotation, and disposal of medicaments
- Support junior members of the nursing team and provide guidance when necessary
- Participate in local initiatives to enhance service delivery and patient care
- Support and participate in shared learning within the practice
We are looking for a candidate with:
- Registered Nurse qualification
- Postgraduate diploma or degree in Primary Care (desirable)
- Experience working in a primary care environment and chronic disease management
- Knowledge of wound care, ECGs, venepuncture, new patient medicals, chaperone procedure, and travel medicine
- Ability to record accurate clinical notes and work within own scope of practice
- Knowledge of health promotion strategies and understanding of evidence-based practice
