As a Community Support Specialist within the Localities team, you will perform assessments, manage intricate cases, and deliver high-quality care. About Our Organization
The Adult Social Care department is undergoing a significant transformation to meet the increasing demand for our services. We are implementing comprehensive changes to enhance care for adults and foster positive working environments for our staff. Our leadership team comprises innovative thinkers who are driving meaningful changes throughout our service delivery.
Currently, we have four Locality teams aligned with the Primary Care Network (PCN) footprint, catering to older adults, individuals with physical and sensory disabilities, and those with mental health needs. These teams facilitate collaboration between Community Support Specialists and NHS colleagues, promoting holistic and integrated approaches.

We expect suitable candidates to meet the following criteria:
- Registered with Social Work England and possess a degree in Social Care or a related field.
- Experience working in a multidisciplinary team environment.
- Proficient in assessing and managing risks while supporting residents.
- Strong knowledge and ability to provide social support to service users and their families.
- Capable of articulating complex social issues clearly to both clients and professionals.
- Adept at planning and coordinating social care plans and aspects of patient discharge.
