About the Role

We are seeking a highly skilled and compassionate Mental Health Practitioner to join our Home Treatment Team in Lancaster. As a key member of our multidisciplinary team, you will play a vital role in providing comprehensive assessments, triage, and onward facilitation to service users in need of urgent mental health support.

Key Responsibilities
  • Provide evidence-based mental health assessments and signposting to alternative service provision for individuals presenting with crisis-level mental health needs.
  • Collaborate with a range of stakeholders, including IRS, Police, and Acute Hospital colleagues, to ensure seamless care pathways and effective triage.
  • Work closely with the Home Treatment Team to provide comprehensive assessment, coordination, and delivery of high-quality mental health services to service users who would otherwise require hospital admission.
  • Contribute to the development and implementation of evidence-based practice, providing clinical advice, support, and education to colleagues as needed.

Requirements
  • Registered Mental Health Nurse, Occupational Therapist, or Social Worker with a strong understanding of crisis planning and contingency planning.
  • Excellent communication and interpersonal skills, with the ability to work effectively with service users, carers, and multidisciplinary teams.
  • Knowledge of care pathways for psychosis and non-psychosis, as well as relevant interventions and motivational skills.
