About the Role:The successful candidate will be responsible for providing community-based responsive input to patients with undifferentiated, varied, and complex needs. You will work across the Rapid Response Team and Home First Team (H1st), ensuring holistic assessment and community care pathway planning to prevent unnecessary acute hospital admission.
Main Responsibilities:- Provide community-based responsive input to patients with undifferentiated, varied, and complex needs.
- Work across the Rapid Response Team and Home First Team (H1st) to ensure holistic assessment and community care pathway planning.
- Develop and implement effective pathways of care through hospital to community.
- Build relationships with hospital and community partners to ensure seamless care transitions.
- Manage acute episodes of care, including cellulitis, urinary tract infection (UTI), falls, functional, mobility, and cognitive impairment.
- Provide interventions and make clinical decisions that support patient choice and wishes.
- Work autonomously while drawing assistance from senior colleagues in primary and secondary care.
- Appropriate professional registration (e.g., NMC, Adult Nursing/HCPC Paramedic).
- Experience of teaching and mentoring students.
- Experience of working in a multidisciplinary team (MDT).
- Medication management experience, including subcutaneous, IV, and IM lines.
- Minimum of 3 years' experience working at Band 5 level.
- Experience of managing acute episodes of care, including cellulitis, urinary tract infection (UTI), falls, functional, mobility, and cognitive impairment.
- Experience of venepuncture.
- Experience of working in an acute setting with high acuity patients.
- Experience of delivering short-term episodes of care (3-7 days).
- Ability to prioritize and manage own workload with minimal supervision.
- Evidence of own Clinical Supervision/Reflective Practice.
- Experience of safe patient handling.
- Contributing to a quality improvement project.
- Community and rapid response experience.
- Experience in advance care planning and End of Life care.
- Experience of leading audit or project.
- Experience of using Patient Record System (e.g., Systm1, Rio).
- Self-awareness, knowing own strengths and weaknesses.
- Contributes towards improving quality in own work area and encourages others to do so.
- Self-management, demonstrating personal resilience.
- Deals with criticism constructively.
- Personal Integrity, having a strong commitment to openness, honesty, inclusiveness, and high standards.
