About the Role

We are seeking a highly skilled and experienced Care Coordinator to join our management team at Island Home Care. As a key member of our team, you will play a vital role in providing exceptional care and support to our clients.

Key Responsibilities
  • Provide holiday cover across Island Home Care, including the additional needs team, approximately 70-80% of your responsibilities.
  • Manage office-based duties, including social work meetings, supervision, care planning, risk assessments, and client assessments, approximately 20-30% of your role.
  • Develop and maintain effective relationships with clients, families, and healthcare professionals.
  • Contribute to the development and implementation of care plans that meet the unique needs of our clients.
Requirements
  • Proven experience in personal care and elderly care.
  • Proficiency in administrative duties, including scheduling, record-keeping, and data entry.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driving license and access to a vehicle.
