Crawley, West Sussex, United Kingdom Sussex Community NHS Foundation Trust Full time

Position Overview

Musculoskeletal Care Coordinator (Patient Support Specialist)

Are you ready to be the initial point of contact for patients seeking care?

The Sussex Community NHS Foundation Trust is in search of exceptional coordinators who can facilitate quality care for our patients. We refer to our coordinators as Patient Support Specialists (PSS).

Patient Support Specialists play a crucial role in the Sussex MSK Partnership service, which assists individuals dealing with musculoskeletal (MSK) conditions. This service guides patients through their care options and offers support for those requiring assessment, diagnosis, and treatment. Our service caters to individuals across various regions with musculoskeletal issues.

Our goal is to simplify the care process for patients by providing a single point of contact as conveniently located as possible.

Key Responsibilities

This is a full-time position ( hours per week) with a standard working schedule of Monday to Friday. We welcome applications from candidates interested in part-time opportunities, and we can accommodate flexible working arrangements, though full-day shifts ( hours per day) are required.

Primary Duties

  • Patient Support Specialists serve as the initial contact for our patients, managing patient information daily and collaborating within small teams to ensure seamless care experiences.
  • Effective communication is essential in this telephone-based role, as you will liaise with local healthcare providers, hospitals, community staff, and clinicians to ensure patients receive timely and appropriate services.
  • Our Patient Support Specialists will alternate their work between the physiotherapy departments at two designated hospitals.

The successful candidate will be employed by Sussex Community NHS Foundation Trust while working within the Sussex MSK Partnership service.

About Our Organization

We are the primary provider of community health services across East and West Sussex. Our dedicated team of 6,000 professionals serves a diverse population, delivering essential medical, nursing, and therapeutic care to both adults and children.

We offer a range of opportunities across our extensive services, including medical, clinical, support, and administrative roles.

Why Join Us?

  • Positive feedback from the NHS Staff Survey, highlighting our commitment to compassionate culture, leadership, and staff wellbeing.
  • Diverse working environments, including community settings, patient homes, and various facilities across the region.
  • Supportive and flexible working arrangements, including part-time, flexible hours, and retirement options.
  • Comprehensive training and development opportunities.
  • Research initiatives.
  • Affordable workplace nurseries available in select locations.
  • Active staff networks promoting diversity and inclusion.
  • Accredited as a Disability Confident Leader, dedicated to fostering inclusive workplaces.
  • Recognized as a Veteran Aware Trust, achieving accreditation from relevant healthcare alliances.
  • Located in beautiful Sussex, with convenient access to natural landscapes.
  • Access to a variety of benefits and discounts for NHS staff.
