About the Role:

We are seeking a dedicated and compassionate Tenancy Support Coordinator to join our team at Change Grow Live. As a Tenancy Support Coordinator, you will play a vital role in supporting individuals who are at risk of becoming homeless and require assistance to maintain or start new tenancies.

Key Responsibilities:
  • Establish effective communication and referral pathways for individuals at risk of homelessness and in contact with other health and care agencies.
  • Provide wrap-around support in emergency/temporary/supported accommodations.
  • Prioritize caseload depending on crisis intervention and severity of needs.
  • Attend relevant multi-disciplinary team meetings to support links to health and social care services, peer support networks, community assets, and mutual aid.
  • Work closely with the local authority, Health & Social Care sector, partners, and any provision linked to preventing homelessness and improve joint working.
  • Increase and enhance partnership working with primary care, mental health services, housing, and other voluntary organizations supporting individuals at risk of losing tenancies.
  • Provide wrap-around support on budgeting, education, training, and employment while continuing to support engagement in structured services.
About You:
  • An understanding and knowledge of the harmful effects associated with drug and alcohol use in relation to health, social welfare, housing, employment, and personal relationships.
  • A minimum of 12 months experience facilitating successful tenancy sustainment and working to achieve positive community connections.
  • Knowledge of benefits and applying for benefits in a supportive role.
  • The experience of engaging with a wide variety of individuals with a variety of backgrounds and needs.
  • The ability to work with and manage challenging behavior.
  • Knowledge of local services and relevant interventions to support and sustain recovery, especially related to training and education.
  • Knowledge of local registered social landlords and tenancy issues that affect our service user group.
