Position Overview
ROLE SUMMARY
We are pleased to announce an opportunity for a Nursing Associate Trainee to become part of our vibrant team at Primary Integrated Community Services.
This role will involve collaboration with a Community Practice Nurse, focusing on delivering care to patients who are unable to visit healthcare facilities. The successful candidate will also partner with the Learning Disability Nurse Practitioner to assist underserved patients in accessing essential healthcare services.
As a Nursing Associate Trainee, you will be supported in pursuing the Nurse Associate foundation degree at a university through an apprenticeship program.

Job Responsibilities
POSITION PURPOSE
Within the Primary Care Network, the selected candidate will provide high-quality, safe nursing care to the community. Under the guidance of the Lead Nurse, Registered Nurses, GPs, and Practice Management teams, they will deliver care in accordance with organizational policies and protocols, focusing on supporting patients with their health needs, promoting self-care, and managing long-term conditions.
Key Responsibilities
Acting as a Responsible Professional

- Collaborate with the multidisciplinary team to provide and monitor care.
Promoting Health and Preventing Illness
Assist patients in enhancing their mental, physical, and behavioral health through:
- Behavior change interventions during health assessments.
- Monitoring chronic diseases such as respiratory issues, diabetes, and hypertension.
- Encouraging participation in health screenings.
- Caring for individuals with complex health conditions.
Delivering and Overseeing Care
Conduct clinical procedures safely within the PCN practices and patients' homes, including:
- Clinical observations (e.g., blood pressure, pulse, temperature).
- Venepuncture and spirometry.
- Wound care and medication administration.
- Assisting with minor surgical procedures.
Contribute to ongoing assessments and recognize when referrals are necessary.
Collaborating Within Teams
- Adapt to changing priorities and seek guidance as needed.
- Support and mentor undergraduate student nurses and new care staff.
- Participate in team meetings and contribute to quality improvement initiatives.
Enhancing Safety and Quality of Care
Uphold health standards by applying infection prevention principles and maintaining a safe work environment.
Contributing to Integrated Healthcare
- Maintain accurate records and effective communication regarding patient care.
- Foster relationships across the multidisciplinary team to support integrated care.
- Identify and manage risks associated with clinical activities.
Supervision
- Ensure access to appropriate clinical supervision and support from designated individuals.
