Position Overview

ABOUT THE POSITION

We are thrilled to announce an opportunity for a Nursing Associate Trainee to become part of our vibrant team at Primary Integrated Community Services.

The role will encompass working collaboratively with a Community Practice Nurse, providing essential care to patients who are unable to visit healthcare facilities. The successful candidate will also engage with the Learning Disability Nurse Practitioner to assist underserved patients in accessing healthcare services. Additionally, the role may involve direct participation within various practices to support the delivery of nursing services as part of a cohesive practice team. Flexibility will be crucial to meet the diverse needs of our service.

As a Nursing Associate Trainee, you will receive support to pursue the Nurse Associate foundation degree at a University through an apprenticeship program.

Job Responsibilities

ROLE OBJECTIVE

The Nursing Associate Trainee will deliver high-quality, safe nursing care to the patient population, supported by experienced nurses and healthcare professionals. The role will involve working within established protocols and policies to address health needs, promote self-care, and engage in health prevention and screening activities.

KEY DUTIES

Health Promotion and Illness Prevention

Support patients in enhancing their mental and physical well-being. Engage in chronic disease management and health screening initiatives. Provide care for individuals with complex health conditions.

Care Delivery and Monitoring

Conduct clinical procedures safely within various settings. Perform essential clinical observations and administer medications as required. Assist healthcare professionals with minor procedures.

Team Collaboration

Recognize and adapt to changing priorities within the team. Mentor and support new staff and students in their roles. Participate in team meetings and contribute to service improvements.

Safety and Quality Improvement

Adhere to infection prevention protocols and maintain a safe working environment.

Integrated Care Contribution

Maintain accurate records and effective communication with healthcare professionals. Build relationships across various healthcare sectors to support integrated care.

Supervision and Support

Ensure access to appropriate clinical supervision and support.
