Position Overview
We are seeking a dedicated, skilled, and motivated Primary Care Nurse Specialist to join our esteemed Nursing team.
This role is part-time, approximately 20-25 hours per week across 3 days, with some flexibility regarding total hours and working days. Candidates are encouraged to specify their preferred hours and working arrangements in their application.
Key Responsibilities
The ideal candidate will possess prior experience in primary care nursing, including standard treatment room procedures, immunizations, cervical screenings, wound care, and chronic disease management. Please provide details of your primary care experience in your application.
We expect professionalism, courtesy, and efficiency, along with exceptional interpersonal skills, enthusiasm, and the ability to perform under pressure.
Main Nursing Duties
- Deliver nursing care to patients as required, adhering to clinical guidelines, NICE recommendations, and local protocols.
- Assess wounds and minor injuries, apply dressings, and refer to a medical practitioner or hospital as necessary.
- Conduct swabs and cervical screenings.
- Administer immunizations and injections to both children and adults.
- Assist in Chronic Disease Management, utilizing the principles of Making Every Contact Count (MECC) to optimize patient interactions and reduce visits.
- Support the Nursing team in managing clinical emergencies.
- Monitor patients' blood pressure and determine the need for referrals back to the physician.
- Assist with ECGs and communicate findings to the medical team.
- Support phlebotomists with venipuncture procedures.
- Insert pessaries as directed by the physician.
- Assist with INR clinics, including patient dosing using INRstar when a suitable Health Care Assistant is unavailable.
Chronic Disease Management
- Maintain chronic disease registers.
- Identify, manage, and support patients at risk of developing long-term conditions to prevent adverse health outcomes.
- Manage conditions such as Coronary Heart Disease (CHD), Hypertension, Smoking Cessation, Diabetes, Asthma, and Chronic Obstructive Pulmonary Disease (COPD).
- Implement and evaluate individualized treatment plans for chronic disease patients.

General Duties
- Uphold high standards of nursing care for patients and promote overall health and well-being.
- Assist physicians during consultations, including chaperoning and urgent ECGs.
- Ensure cleanliness and hygiene in the nursing area and maintain clinical equipment.
- Monitor and maintain adequate supplies of medications and clinical items.
- Record all treatments accurately in patient records using the computer system.
- Maximize the use of templates and read codes effectively.
- Engage in health promotion initiatives and utilize relevant resources for patient education.
- Participate in training and development opportunities as required.
- Support patients in understanding their prescribed medications.
- Stay updated with training needs and professional registration requirements.
Health Promotion
- Develop and implement health promotion and wellness programs.
- Discuss lifestyle factors such as diet, alcohol consumption, and smoking with patients.
- Provide family planning and travel advice.
- Assist with health checks and monitor cervical cytology results.
Managerial Responsibilities
- Collaborate with physicians and management to enhance patient services.
- Ensure compliance with GMS Contract requirements within the nursing department.
- Participate in audits and inspections as required.
- Assist in maintaining accurate clinical histories.
- Support and supervise Health Care Assistants.
Communication Skills
- Demonstrate strong teamwork and attention to detail.
- Provide guidance to non-clinical staff on clinical matters.
- Communicate effectively with patients and other healthcare professionals.
Qualifications
Essential
- NMC registration.
- Experience in primary care and treatment room settings.
- General nursing experience.
Desirable
- Experience working in a GP practice.
- Chronic Disease Management qualifications.
