We are seeking a highly skilled and motivated Community Nurse Specialist to join our Out of Hours team at Torbay and South Devon NHS Foundation Trust. As a key member of our team, you will play a vital role in providing high-quality care and support to patients in their own homes.
Key Responsibilities- Provide Holistic Nursing Care: Conduct comprehensive assessments, develop and implement individualized care plans, and provide high-quality nursing interventions to patients in their own homes.
- Lead and Manage the Team: Supervise and manage a team of Community Nurses and SNRs, ensuring that they have the necessary skills and support to deliver excellent patient care.
- Collaborate with Healthcare Teams: Work closely with hospital, practice, and crisis response teams, as well as community matrons and Intermediate Care services, to ensure seamless care coordination and patient outcomes.
- Develop and Implement Care Plans: Work with patients, carers, and healthcare teams to develop and implement care plans that meet the individual needs of patients, promoting independence and quality of life.
- Contribute to Quality Improvement: Participate in quality improvement initiatives, audits, and research to enhance patient care outcomes and contribute to the development of best practices within the team.
- Professional Qualifications: Possess a relevant nursing qualification, such as a Community Specialist Qualification, and a Non-Medical Prescriber Qualification.
- Experience and Skills: Have significant experience in community nursing, with a strong focus on patient-centered care, and possess excellent communication, interpersonal, and leadership skills.
- Continuous Professional Development: Demonstrate a commitment to ongoing professional development, staying up-to-date with best practices, and contributing to the development of the team.
