Job Title: General Practice Nurse Trainee

Job Summary:

We are seeking a highly motivated and enthusiastic General Practice Nurse Trainee to join our team at Hammersmith and Fulham Training Hub CIC. As a General Practice Nurse Trainee, you will have the opportunity to work in a dynamic and supportive environment, providing high-quality care to patients and developing your skills and knowledge in General Practice Nursing.

Key Responsibilities:

  • Provide high-quality patient care, working closely with the General Practice team to deliver excellent patient outcomes.
  • Develop and maintain strong relationships with patients, families, and carers, ensuring that their needs are met and their expectations are exceeded.
  • Participate in the development and implementation of care plans, working collaboratively with the multidisciplinary team to ensure that patients receive comprehensive and coordinated care.
  • Contribute to the maintenance of accurate and up-to-date patient records, ensuring that all relevant information is recorded and communicated effectively.
  • Participate in ongoing professional development, staying up-to-date with the latest clinical guidelines, policies, and procedures.

Requirements:

  • Up-to-date membership of the NMC, preferably adult nursing branch.
  • Qualification at Diploma level in Adult Nursing - University entry criteria varies.
  • No GPN experience is necessary for this training post. You can apply as a Newly Qualified RN. Equally, you can transition from another area of nursing as long as you have qualified as an Registered Adult Nurse.

Working Relationships:

  • Internal: working with all clinical and non-clinical staff in the General Practice team, accountable to the Lead General Practice Nurse and GP employers.
  • External: working with other practices in local networks and with the Training Hub and Clinical Commissioning Group (CCG) at a Borough level.

Key Result Areas:

  • Positively impacting the public health of patients and residents via vaccinations, screening, etc.
  • Providing care to those with complex and chronic disease physical and mental problems, enabling them to stay well and stay out of hospital.
  • Supporting families and carers who are caring for those with mental or physical problems and distress.
  • Maintaining and improving excellence within the primary care team.

  • It is your responsibility to adhere to the General Practice infection control policies and guidelines in order to promote cleanliness and reduce infections.

  • You are expected to comply with all General Practice policies and procedures related to confidentiality and data protection and to work in accordance with relevant data protection legislation.

Risk Management:

  • You are expected to be familiar with the General Practices risk assessment and risk control processes and procedures and to report accidents or incidents promptly.

Conflict of Interest:

  • You are required to declare any interest, direct or indirect, with contracts involving the General Practice and should not further your private interests during your work.

  • If you have responsibility for a budget, you are expected to operate within this and under the General Practice policies and guidance.
