As a Community Wellbeing Support Specialist, you will work closely with patients referred from our Primary Care Network (PCN) Core Network Practices. Your primary responsibility is to provide personalized support, guiding individuals towards accessing a wide range of non-medical community-based activities and services tailored to their practical, social, and emotional needs.

You will utilize coaching and motivational interviewing techniques to encourage self-management and promote a holistic approach to health. Your expertise will extend to collaborating with local organizations, including Voluntary Community Social Enterprise (VCSE) groups, authorities, and other stakeholders to identify gaps in community provision and foster sustainable growth in support services.

Key Responsibilities:

  • Receive referrals from PCN Core Network Practices and other relevant sources.
  • Develop personalized care and support plans with individuals, guiding them towards appropriate community activities, groups, and services.
  • Manage your caseload effectively under the supervision of the PCN team, prioritizing needs and ensuring timely support.
  • Collaborate with other healthcare professionals and agencies as needed to provide comprehensive care.
  • Build strong relationships with local organizations and community support services to enhance knowledge of available resources and foster effective partnerships.
  • Empower local VCSE organizations and community groups by facilitating referrals and promoting their role in the social prescribing network.
  • Educate clinical and non-clinical staff within PCN Multidisciplinary Teams (MDTs) about the benefits of community-based interventions and how patients can access them.
  • Promote social prescribing as a valuable approach across the PCN and wider healthcare system, emphasizing its role in self-management, addressing health inequalities, reducing pressure on statutory services, and improving overall health outcomes.
