Position Overview
An exceptional opportunity has emerged within Southern Health NHS Foundation Trust for a dedicated and passionate Lead Clinical Nurse to become a vital part of the out-of-hours nursing team. The successful candidate will need to demonstrate flexibility, as the role may involve a combination of twilight and occasional night shifts. The twilight hours are from 17:00 to 23:30, while night response shifts run from 22:00 to 04:30. The position is structured around an 18-hour contract, encompassing three nights or twilight shifts per week, with the potential for additional hours based on service demands.
The out-of-hours response teams are responsible for covering various local GP practices, ensuring comprehensive care across the community. Candidates should be prepared for the possibility of working at different locations as needed to maintain service quality.
In this role, you will collaborate closely with an experienced auxiliary nurse, addressing the diverse needs of clients in the community. The responsibilities are varied and engaging, including but not limited to end-of-life care, fall prevention strategies, and catheter management. You will be directly responding to client needs and coordinating with local community care teams. Additionally, you will engage with out-of-hours GP services, SCAS, and 111 to facilitate timely hospital discharges and prevent unnecessary admissions, thereby supporting our urgent care initiatives and the virtual ward.
Access to a vehicle and possession of a valid driving license are mandatory for this position.

The Integrated Care Team model in East Hampshire aims to deliver a responsive, safe, and effective service. The team is designed to meet the needs of the local population, focusing on providing a local access point, proactive case management, and a collaborative approach with patients and service users. Operating from a local hub, the Integrated Care Team ensures a cohesive approach across the designated locality, striving for excellence in care while engaging with all practices and stakeholders in the area.
