Job Summary

We are seeking a highly motivated and experienced Community Mental Health Nurse to join our Memory Services team at South West Yorkshire Partnership NHS Trust. As a key member of our team, you will be responsible for providing high-quality care and support to individuals with cognitive impairment and their families.

Key Responsibilities

  • Conduct comprehensive assessments and develop personalized care plans to meet the unique needs of each individual.
  • Provide expert care and support to individuals with cognitive impairment, including those with dementia and other related conditions.
  • Work collaboratively with multidisciplinary teams to ensure seamless care coordination and delivery.
  • Develop and maintain strong relationships with individuals, families, and carers to promote empowerment and independence.
  • Stay up-to-date with the latest research, best practices, and guidelines in memory care to ensure evidence-based practice.

Requirements

  • Registered Mental Health Nurse with a valid NMC registration.
  • Experience working with individuals with cognitive impairment and their families.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders.
  • Ability to work independently and as part of a team, with a flexible approach to meet the needs of the service.
  • Valid driving license and access to a car for community travel.
