We are seeking a skilled and experienced support worker to join our team at Community Integrated Care. As an Advanced Support Worker, you will play a vital role in supporting adults with learning disabilities and autism, who have complex needs, to lead the best life possible.

About the Role

This is an exciting opportunity for a dedicated and passionate individual to make a real difference in the lives of our service users. You will be working in a supported living project for 4 young gentlemen with complex care needs, providing person-centred support to help them settle into their new lives.

Key Responsibilities
  • Provide empathic and patient support to individuals with complex care needs
  • Support people to work towards person-centred outcomes, enabling them to develop independence and maintain safety in their home and community
  • Work collaboratively with the core background team to provide 24/7 support

Requirements

To be successful in this role, you will need:

  • Experience of supporting people with complex care needs, youth transition, and supporting young adults who are care leavers
  • Excellent communication and interpersonal skills
  • A patient and empathic approach to supporting individuals with complex needs
