Position Overview
Are you a dedicated Physiotherapist seeking a rewarding opportunity within a community-focused environment? In this role, you will play a vital part in assisting individuals in their journey back to independence within their own residences.
We invite you to become a member of our welcoming D2A team, which operates as a segment of the broader Intermediate Care service in Somerset. This position offers you the chance to deepen your understanding of community healthcare and the essential support provided to individuals in their homes.
Thanks to our successful outcomes and continued progress, we have received additional funding to further develop our services, resulting in several new positions becoming available.
Key Responsibilities
Your primary responsibilities will include conducting targeted therapy evaluations, which encompass goal establishment and treatment strategy formulation, risk assessment, and the promotion of independent living. You will embrace a person-centered approach, demonstrating flexibility and the ability to prioritize effectively. Collaboration with our provider teams and rehabilitation professionals will be essential.
Are you a fully qualified Physiotherapist with an active HCPC registration, or are you in the process of obtaining it?

Maintain effective communication and collaborative relationships with other Trust teams, external agencies, and Trusts.
Utilize various communication methods (both verbal and non-verbal) to obtain informed consent from patients, often overcoming barriers such as hearing loss, altered perception, and cognitive challenges. You will be equipped to operate within a legal framework when working with patients who may lack the capacity to consent to treatment.
