We are seeking an experienced Clinical Pharmacist to join our team at Ely PCNs. As a key member of our Primary Care Network, you will play a vital role in supporting our practices and patients with medication management and optimization.
Main Responsibilities- Support our PCN Pharmacy Technicians in their daily tasks and activities.
- Conduct medication reviews and optimize medication regimens for patients with long-term conditions.
- Receive on-the-day referrals for patients needing advice and guidance, and referrals from practice colleagues.
- Offer advice and support to patients and colleagues on medication management and optimization.
- Advise on the management of common, minor, self-limiting ailments and over-the-counter medications.
- Provide education and training to practice teams, care home teams, and patient groups on therapeutics and medication optimization.
- Reconcile medicines following discharge from hospitals, intermediate care, and admissions to care homes.
- Identify cohorts of patients at high risk of harm from medicines using clinical databases.
- Undertake clinical audits and analyze, interpret, and present medicines data.
- Summarize and communicate key prescribing updates and medicines alerts.
- Support the implementation of local and national guidelines and formulary recommendations.
- Monitor practice prescribing against local dashboards.
- Support the PCN Network DES.
Ely PCNs is a well-established Primary Care Network serving over 80,000 patients across six GP practices. We are committed to delivering high-quality patient care and improving health outcomes through innovative and collaborative approaches.
Requirements- Registration with the General Pharmaceutical Council.
- Hold or be willing to work towards an Independent Prescribing qualification.
- Minimum of three years' experience as a Pharmacist, demonstrated within a practice portfolio.
- Experience and awareness of common acute and long-term conditions.
- Experience of partnership/collaborative working and building relationships across various organizations.
- Ability to organize, plan, and prioritize workload on own initiative.
- Adaptable and resilient.
- Able to work well under pressure and meet agreed deadlines.
- Excellent written and oral communication skills.
- Ability to maintain effective working relationships and promote collaborative practice with all colleagues.
- Ability to identify risk and assess/manage risk when working with individuals.
